Terms of Service

Thank you for choosing SUPER LASH SPA, LLC. Every precaution will be taken to ensure your safety and well-being before, during, and after your lash extension application, please be aware of the following information and possible risks:

CELL PHONES: The use of cell phones while service is being performed is not permitted. Please put your phone on silent mode during service.

CONTACT LENSES: Please remove/take off any contact lenses prior to beginning eyelash services.

ALLERGIES: If have had any previous allergic reactions to eyelash extensions or glue, please notify your technician before the beginning of your eyelash service.

SAFETY: I understand that in order to have the eyelash extensions applied to my eyelashes I will need to keep my eyes closed for 60-90 minutes. I understand I cannot open my eyes for any reason during the service, because this can cause a disruption to the service and affect the entire eyelash extension, lift/perm or tinting result. Eye irritation, pain, itching, discomfort, and in rare cases – eye infection may occur. I will notify my technician if I experience discomfort associated with my eyelashes. My technician will assess the situation and may stop the procedure if necessary. I may also request to stop the procedure. I understand and agree that if I experience any of these issues with my lashes after the procedure, I will contact my technician immediately and consult a physician at my own expense.

SHEDDING AND REFILLS: I understand that eyelash extensions are a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally. Losing 1-5 eyelash extensions per day is normal. To maintain and keep a full look, a touch-up is recommended every 2-4 weeks, this will vary depending upon your own personal aftercare maintenance, hygiene, and the shedding cycle of your own natural lashes. Eyelash Extension Refills mean: technicians will add in new eyelash extensions in spots I have no extensions; The refill procedure may remove some old extensions and replace them with new ones. A new set of lash extensions is needed many times INSTEAD OF A REFILL. Too many refills without proper cleaning, excess makeup use, and other factors may easily add bacteria and cause general wear to your eyelashes. Your Lash Technician will advise you at the beginning of your eyelash extension service what we are able to offer for your lashes.

RESULTS: I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially wanted. The results may vary depending on the thickness and length of your natural lashes, SUPER LASH SPA LLC. has extensions of different weights, lengths, thicknesses, and materials to complement and enhance your natural. The eyelash extensions will be applied to my natural lashes. The weight and length of the lashes are determined by the lash technician in an effort not to create excessive weight on the natural eyelash, thereby preserving the health, growth, and natural look of the client’s natural lashes.

CANCELLATION POLICY: I understand that in order to avoid any cancellation charges, I must cancel or reschedule my appointment at least 8 hours prior to my appointment time. Late arrival of more than 15 mins without prior notification by a phone call or text message, will be canceled automatically and a cancellation fee will be applied.

AFTERCARE: I understand that I will be advised to follow the aftercare protocol by my technician. The aftercare protocol is designed to avoid discomfort or adverse side effects after the procedure has been completed. I agree to follow the aftercare instructions provided by my technician.

PERMISSION OF PHOTOGRAPHY AND USE OF IMAGES: I give permission to SUPER LASH SPA, LLC to take “before and after” photographs for the purpose of documentation, advertising, and promotional purposes.

WARRANTY AND PRICING: I understand that the warranty covers the specific eyelash extensions or refill work we performed under reasonable use, for 3 days. Service under warranty is provided in our shop only. We will not be held responsible for lost time, wages, or any other losses related to our products or services. If the problem is due to improper aftercare by the client there will be a charge for the repair. The charge will be determined at the time of the lash repair. NO Refunds after service is completed. All Sales are Final. Please refer any questions or issues regarding your services provided to your Eyelash Technician or store manager immediately after service is complete. Coupon discounts or promotional pricing can only be used once per new client. Existing clients cannot use promotional pricing or coupons.
